Dr. Caine, the deadly dentist from the first movie now escapes from the mental hospital with a hope of resuming a normal life. He travels to a quiet Midwestern town under a fake name and takes on the responsibilities of the town dentist. As he finds out his new love in the arms of someone else, he sends him back over the edge and into another homicidal rampage, with his unlucky patients bearing the brunt of his hostility.
